What to Expect
Once you schedule an appointment, you will be sent a link to create a portal in Therapy Notes. You will be asked to complete the following forms:
HIPAA
You have to sign a HIPAA (Health Insurance Portability and Accountability Act) form to acknowledge your understanding of how your protected health information (PHI) may be used and disclosed by healthcare providers or insurers. It ensures your privacy rights are protected and outlines how your medical information can be shared for treatment, payment, and healthcare operations purposes.
Identification
We will need to take a copy of your identification card to verify your identity, ensure accurate record-keeping, facilitate billing and insurance processing, and maintain security and confidentiality of personal and medical information.
Insurance Card
We will need to take a copy of your insurance card to verify insurance coverage, ensure proper billing, facilitate claims processing, and determine eligibility for specific services and benefits.
Financial Arrangements
We require that all clients sign this form to acknowledge they understand our payment policy.

If you are not able to complete the forms or have difficulty with technology, please let the office staff know as soon as possible. Completing paperwork in session will cut into your appointment time and we would like to prevent that if possible.
