Payment

About Payments

If you are not using insurance, payment is appreciated at time of service. When you schedule your initial appointment, you will be sent a No Surprises Form that provides rates and an estimation of the total cost. The actual cost may vary depending the on the length of time needed to adequately address your concerns.

Notes on Health Insurance

Most insurance plans cover psychotherapy if it is deemed to be medically necessary. To fit this criteria, you must have a diagnosis (this will be assessed for in your first visit) that impacts important areas of life such as employment, education and/ or relationships.

If you want to use insurance, please complete the insurance form on the portal at least two business days ahead of your intake appointment. Our biller will look up your benefits on the insurance portal enabling us to give you an estimate of cost at your first appointment. Often your coverage for psychotherapy will be similar to coverage for primary care visits. For example, if you have a $25 copay for a primary care visit, you will often have a $25 copay for therapy. If you have a deductible to meet, your cost per session will vary depending on your insurance contract. If you have questions about your cost, you can call your insurance (the number is usually located on the back of your card) and ask them the contracted rate for psychologists (Dr. Roth, Dr. Neely) or social workers (Elizabeth McCormack, Mike Morey) for an intake appointment (CPT code 90791) and therapy appointments (CPT codes 90837 or 90834 depending on session length).

We accept many insurance plans including:

  • Medicare
  • Priority Health
  • Blue Cross Blue Shield
  • Blue Care Network
  • McLaren
  • Aetna
  • Tricare East

If a client misses an appointment without one business day notice, they may be responsible for the entire session fee.

We accept cash, check or credit card as forms of payment. If you are using a credit card, you will have the option of keeping it securely on file in our electronic health record and we will run it after your appointment. If you choose not to keep a card on file, you can use the portal to make a payment. Accounts with balances will be sent a paper bill monthly.

To ensure access to therapy and maintain financial stability, our office requires that all outstanding bills be addressed or a payment plan be established before scheduling appointments. This policy helps us continue providing quality therapy while supporting a clear understanding of financial responsibilities between clients and our practice. We appreciate your cooperation in maintaining this important aspect of our client care process. For assistance with payment arrangements or any questions, please call us at 231-340-9113.
